Report of Trouble 

A report of trouble informs about a trouble or a hindrance in the operation of a machine or in completion of a work, routine job or a project or the like. It explains the nature of the problem or hindrance and possible solution also. It may be presented in memo or letter or manuscript format depending upon the nature of the problem.
